Purpose:
This role has responsibility for developing, providing, monitoring, and maintaining treatment and service plans for residents and their families.
Duties & Responsibilities:
-Develop treatment plans for residents and their families
-Develop service plans for residents and their families
-Update treatment and service plans
-Monitor treatment and service plans for progress and completion
-Work with the residents and their families
-Develop a visitation schedule for resident and family
-Develop a plan for functional independence for residents 14 years and older
-Establish goals for the residents and their families

Required Education & Experience:
A social service worker, at the time of appointment shall possess the following:
A bachelor's degree in a human behavioral science or any other major with 25% of the credits in a human behavioral science from an accredited college or university

-Must not be listed on Central Registry for the abuse or neglect of a child
-Must not be listed on the Sexual Offender Registry
-Must be of good moral character
